Laparoscopic Treatment of Hydrocele of the Canal of Nuck in Female Adults: Laparoscopic Intracorporeal Hydrocelectomy and Posterior Wall Suture Repair

Abstract
Purpose: Hydrocele of the canal of Nuck (HCN) causes inguinal swelling and pain in women. This study aimed to evaluate the outcomes of laparoscopic intracorporeal hydrocelectomy and posterior wall suture repair for HCN treatment.

Methods: We retrospectively reviewed the charts of 90 adult female patients with HCN who were admitted at Damsoyu Hospital, Seoul, Republic of Korea, from September 2012 to July 2019. Laparoscopic intracorporeal hydrocelectomy and posterior wall suture repair were performed in all patients.

Results: Of the 90 patients, encysted hydroceles were observed in 73 (81.1%) and communicating hydroceles were observed in 17 (18.9%). Among the 73 patients with encysted hydroceles, 63 (86.3%) had serous hydroceles, 9 (12.3%) had hemorrhagic hydroceles, and 1 (1.4%) had an inflammatory hydrocele. Twenty patients complained of inguinal pain only in the encysted hydrocele, and the pain was significantly higher in patients with hemorrhagic hydroceles (P<0.001). Among patients with encysted hydroceles, 45 (61.6%) patients had complete patency of the processus vaginalis. There was one complication of hematoma in the inguinal area. No recurrence was observed during the follow-up period.

Conclusion: HCN should be considered in the differential diagnosis of inguinal hernia in women with inguinal swelling. Preoperative ultrasonography should be used in the diagnosis, and laparoscopic intracorporeal hydrocelectomy with posterior wall suture repair can be performed as an effective treatment
